What causes some menues not to be seen by nvda?

Hi every so often I get a situation, only fixable by a complete reboot of
windows 7 in which no context menus or nvda menus are spoken. as can be seen
below opening on in Explorer, for example seeems to suggest it does not get
focus, but why should these menus  be different to normal menus?
 Is there something going on with nvda that upsets windows, or is this a
known issue that windows sometimes has with what it is sending to nvda?
 Here is a log from opening an explorer window and attempting to open the
context memu
